We support individuals, both children and adults, with disabilities and other barriers to community living by providing independent living assessments, skill-building services, and respite services to help them thrive at home and in the community.

We’re currently hiring a dedicated and motivated Community Support Coordinator to join our team, serving participants in Wood and Portage Counties. This role involves daily travel throughout the region, engaging directly with individuals from ages 5-99 to assess needs, teach life skills, and coordinate supportive services.

Key Responsibilities:

  • Conduct independent living assessments in participants’ homes and community settings
  • Teach practical life skills based on individual needs
  • Coordinate services with local community agencies and referral partners
  • Provide case management including scheduling, documentation, and progress reporting
  • Collaborate effectively with assigned work teams and agency partners
  • Empower individuals to set and achieve goals for greater independence

What We’re Looking For:

  • Must be available for days, some nights and some weekends
  • Prior experience working with individuals with disabilities or in service coordination (preferred, but not required – we’ll train the right candidate!)
  • Strong communication, organization, and problem-solving skills
  • Self-motivated and able to manage your own workload
  • Valid driver's license, clean driving record, and insured vehicle for daily travel
  • Bachelor’s or Master’s degree preferred (Compensation based on education and experience)
